Golden Protection Personal Accident Insurance is available to individuals aged between 19 to 70 on their next birthday, must be in normal health and free from any physical deformity. 2. This insurance plan excludes those who travel with motorcycles while on duty. 3. It will not cover those who have lapsed their policy or stopped paying their premiums. 4. Premiums paid are not refundable if you cancel your policy. 5. This plan has a waiting period of seven (7) days. You will not be compensated if accident happened within seven (7) days from the date of your 1st policy issued. 6. This insurance plan covers accident happened worldwide except Afghanistan, Cuba, Democratic Republic of Congo, Iran, Iraq, Liberia, Sudan or Syria. 7. You won’t be compensated for any event directly or indirectly arising out of: 1. War, civil war, invasion, insurrection, revolution, use of military power or usurpation of government or military power. 2. Martial law or state of siege or any of the events or causes which determine the proclamation or maintenance of martial law or state of siege. 3. Deliberately self-inflicted injury or suicide or the criminal or illegal act. 4. Fits, hernia, illness of any kind venereal disease including those relating to the Acquired Immune Deficiency Syndrome (AIDS), pregnancy, childbirth, miscarriage, confinement or any complications thereof, intoxicating liquor, drugs or misuse of drugs prescribed by a legally qualified medical practitioner, suicide, self-inflicted injury, judicial pronouncement, unlawful act on the part of the Participant or wilful exposure of the Participant to unnecessary danger except in an attempt to save human life. 5. While the insured is in a state of unsound mind, drugs overdosed, gas inhalation and/or suffocation, food and/or drink poisoning, provoked and/or unprovoked assault or murder, hijacking and/or disappearance. 6. Any act of any person acting on behalf of or in connection with any organization with activities directed towards the overthrow by force of any de Jure or de facto Government or to the Influencing of it by terrorism or violence. 7. While the Participant is using power-operated wood working machinery or engaging in Hunting, Trekking activity in any place with height more than four thousand (4,000) meters above sea level, Mountaineering, Steeple chasing, Polo, Racing of any kind, riding a two-wheel motorcycle, Water Skiing, Ice or Winter Sports of any kind, Scuba diving, Skin diving of any kind including the use of Aqua-lungs, Boxing, Wrestling and Training or performing any form of Martial Arts and all aerial sporting activities unless previous consent of the Operator has been obtained and the Certificate has been endorsed accordingly. In any event whatsoever any person engaging or participating in any sport on a professional basis, will not be covered under the Certificate. 8. Participating in or training for any hazardous or dangerous sport or competition or riding or driving in any form of race or competition. 9. Sickness, disease or any kind of infection however contracted, even if through injury and/or snake, insect and/or animal bite. This Exclusion however, does not apply to sickness or disease directly resulting from medical or surgical treatment rendered necessary by an injury or to infection directly resulting from an injury, provided that in each case the injury itself is covered by this policy. 10. The insured person having a blood alcohol content over the prescribed legal limit when driving or operating any motor vehicle, and/or being under the influence of intoxicating liquor and/or being under the influence of any drug other than a drug taken or administered by, or in accordance with the advice of, a legally qualified medical practitioner. 11. While the Participant is in on or ascending into or descending from any aircraft other than a fully licensed commercial passenger carrying aircraft in which the Participant is travelling as a passenger (on scheduled flights only) other than as a member of the crew and not for the purpose of undertaking any trade or technical operation therein or thereon. 12. By or contributed by or arising from ionizing radiation or contamination by radioactivity from any nuclear fuel or from any nuclear waste from the combustion of nuclear fuel. 13. By or arising out of or consequent upon or contributed to by HIV infection, Acquired Immune Deficiency Syndrome (AIDS) or AIDS Related Complex (ARC) howsoever this syndrome has been acquired or may be named. 14. Any action caused or an injury inflicted by any person deriving benefit from this insurance policy or by any person conspiring with any person deriving benefit from this insurance policy. 15. Importance of making a nomination for your insurance policy in Malaysia. Introduction This is an introductory guide to help you understand the importance of making a nomination in your life insurance policy to safeguard the interest of your loved ones. It gives you some basic information to help you understand the nomination process and its benefits. A brief explanation on several technical terms used is also provided to help your reading process. Making a nomination The purpose of having life insurance is to ensure that your loved ones are protected financially should anything happen to you. It is then important that your loved ones can access the funds quickly. If you do not make a nomination in your insurance policy, your insurance company is not obligated to release the policy moneys until your loved ones obtain a Grant of Probate or Letter of Administration or Distribution Order, which may take several years. However, if a nomination is made, the policy moneys can be disbursed much faster without the need to obtain the Grant of Probate or Letter of Administration or Distribution Order. In addition, in the case of a non-Muslim policy owner, a trust will be created in favour of the nominee under the following circumstances: • if the nominee is the spouse or child of the policy owner, or • if the nominee is the policy owner’s parent (provided that there is no living spouse or child at the time of such nomination). When such a trust is created, the policy is known as a trust policy. As a trust policy, the policy moneys payable do not form part of the estate of the deceased policy owner and are not subjected to the deceased policy owner’s debts. This ensures that your loved ones get the full amount of the policy moneys payable. However, this is only applicable if the policy is taken in good faith and with no intention to defraud creditors. A trust policy does not apply in the case of a Muslim policy owner. The nominee of a Muslim policy owner takes the policy moneys only as an executor and must distribute the moneys in accordance with Islamic laws.....

Nomination is important to ensure that your loved ones will receive the protection as intended by you and that policy moneys may be paid out by insurers without any delay. Where no nomination is made, the policy moneys will be paid out to your lawful executor (if you have made a Will) or your lawful administrator (if no Will has been made) – this process will take a while. *If there is no lawful executor or administrator, then the policy moneys will be paid out to the rightful persons in accordance with applicable distribution laws. 2. Who can I nominate? You can nominate any individual(s) you want. HOWEVER, not all nominees will receive the policy moneys beneficially or for their own use. Depending on your marital status and who you nominate, nominations may produce unexpected results for the nominee(s). Likewise, nominations by a Muslim policy owner and non-Muslim policy owner will have different consequences. How so? Nominations made by a non-Muslim policy owner will create either (i) a trust policy in favour of the nominee(s) or (ii) a non-trust policy. To ensure that your nominee(s) receive the policy moneys as intended, you need to ensure that a trust policy is created in favour of your nominee(s). 3. How do you create a trust policy? Trust policy will be created: (a) if the nominee is your spouse or child; or (b) if the nominee is your parent (provided that there is no living spouse or child at the time you make the nomination). 4. What if I nominate someone else as my nominee(s)? In such as case, you are creating a non-trust policy. Here, your nominee will only receive the policy moneys as executor and distribute the policy moneys in accordance with your Will or where there is no Will, in accordance with applicable distribution laws. If you intend for these nominee(s) to receive the policy moneys beneficially and not as executor, you will need to assign the policy benefits to them. In brief, if you nominate people other than your spouse, children or parents, make sure to assign the policy benefits to them to ensure that they receive the policy moneys for their own use. The premium that you pay for a PRUhealth plan is determined by your age, occupation class, plan type and coinsurance / deductible option chosen, subject to underwriting. PREMIUM PAYMENT Premiums can be paid yearly, half-yearly, quarterly or monthly via Auto Debit, Credit Card, Cash or Cheque. HOW DOES PRUhealth PAY FOR OVERSEAS MEDICAL TREATMENT VERSUS PRUmedic overseas? If you receive medical treatment overseas under your PRUhealth plan, the benefits are paid according to the costs of treatment that would be reasonably charged by a hospital in Malaysia. No benefit is paid if you reside overseas for more than 90 days per trip. However, the 90 days restriction on overseas residence will not be applicable to PRUmedic overseas when attached to your PRUhealth plan. Reimbursement of medical expenses shall also be based on the reasonable and customary charges of the country of treatment. This plan is for people aged between 1 – 70 years old next birthday who want a plan that takes care of their medical expenses AND rewards them for staying healthy. With additional premium and underwriting, you can now enhance the coverage of your PRUhealth plan with: a) PRUannual limit waiver – the rider plan that allows you to make claims higher than your annual limit by waiving the annual limit of your PRUhealth plan. This will ensure that you are safeguarded against any unexpected large medical claims within a policy year. PRUannual limit waiver can only be attached to PRUhealth 200, 300 and 400 plans (subject to additional premium). When PRUannual limit waiver is attached, claim amounts exceeding the annual limit will be subjected to a coinsurance amount of 10% on total claims (excluding cost of daily room & board). b) PRUmedic overseas - have the option to seek overseas medical treatment or advice in China, Hong Kong or Singapore for the following covered conditions: • Surgery related to cancer • Neurosurgery • Coronary artery bypass surgery • Heart valve surgery • Organ transplant (kidney, lung, liver, heart, pancreas and bone marrow transplant) In the event of a covered condition, if you choose to be hospitalised and undergo surgery in Malaysia instead of overseas due to reasons such as family or immobility, a Well-Being Benefit will be payable per admission for every surgery. Policyholders who claim Well-Being Benefit are no longer eligible to reimburse claims for Hospital & Surgical Benefits and Out-patient Treatment Benefits for that particular covered condition. Instead, you may claim Well-Being Benefit for multiple admissions for surgery on covered conditions in Malaysia, and claim for local medical treatment under PRUhealth based on reasonable and customary charges. c) PRUmedic auto upgrade – with this benefit, our customers will have their PRUhealth plan automatically upgraded. On the 5th and 10th rider benefit anniversary, the PRUhealth plan will move up to the next higher plan following the table below: d) PRUmedic retirement - your PRUhealth plan with deductible option will be switched automatically to coinsurance option when you retire at age 55 years next birthday with PRUmedic retirement.

Crisis Defender We will pay you the benefit if you are diagnosed with a critical illness covered under Crisis Defender. In the event of critical illness before age 5 next birthday (n.b.), the proportion of the Crisis Defender sum assured payable is 20%, 40%, 60%, 80% and 100% for age 1, 2, 3, 4 and 5 next birthday respectively. Early Crisis Protector We will pay you the benefit if you are diagnosed with the illnesses, conditions or medical procedures covered under Early Crisis Protector. You are allowed to make multiple claim as long as the sum assured has not been fully paid. Once we have paid a claim, a subsequent claim of the same or lower severity within the same category will no longer be covered. However, for a subsequent claim of higher severity within the SAME category, we will pay you: i. the difference between the relevant amount for the subsequent claim and the amount we paid to you for the previous claim(s); or ii. the reduced amount of benefit at the time the subsequent claim is made; whichever is lower. For a subsequent claim from a DIFFERENT illness category, we shall pay you: i. the relevant amount for the subsequent claim; or ii. the reduced amount of benefit at the time the subsequent claim is made; whichever is lower. Note: We will pay your claims as long as you have survived 30 days after being diagnosed with the illnesses or conditions covered under Crisis Defender and Early Crisis Protector.
